BATHING: Cleaning the body, often including getting in and out of the tub or shower, soaping, rinsing, and drying. DRESSING: Selecting appropriate clothing and putting it on, including buttoning, zipping, and fastening. TOILETING: Getting to and from the restroom, using it correctly, and cleaning oneself afterward, including managing continence. TRANSFERING: Moving from one position to another, such as getting in and out of bed or a chair. FEEDING: Assist with eating and drinking, including reduce risk of aspiration by serving texture -modified food, and utilization of utensils. CONTINECE: Controlling bowel and bladder functions. GROOMING: Maintaining personal hygiene, including brushing teeth, combing hair, and trimming nails. AMBULATING (Functional Mobility): Moving around safely, including walking, getting in and out of bed, and assistance/education on using mobility devices such as walkers and wheelchairs.

NUTRITIONAL MEALS Prior to admission, we review a list of each resident’s food preferences, including favorite meals, to create a personalized menu. Three nutritious meals are served daily, with snacks and beverages available at any time. ROOM: Each room will be equipped with a fully adjustable electric hospital bed and a Smart TV with cable, subscriptions to Netflix, Hulu, and other streaming platforms. LAUNDARY SERVICES: Includes assistance with washing, drying, and folding clothing.

MEDICATION RECONCILIATION: Verifying and clarifying medication information: Ensure accuracy of the gathered information, including dosages, frequencies, and routes of administration. ORDERING MEDICATION/REFILL: Obtaining a Prescription (if required) or requesting a refill prior to running out of medications. ADMINISTERING MEDICATIONS: Adhering to the "Five Rights" of medication administration: Ensuring the right patient receives the right drug, in the right dose, via the right route (e.g., oral, injection, topical), & at the right time. Monitoring the patient during and after administration: Observing the patient for any adverse reactions or side effects and report to physician. PATIENT EDUCATION AND COUNSELING: Providing patients with clear and understandable information about their medications, including how to take them, potential side effects, and assist in reaching out to physician if they have concerns MEDICATION STORAGE & DISPOSAL: Proper storage of medications: Storing medications in a safe, secure, and appropriate environment to maintain their efficacy and prevent accidental ingestion. Safe disposal of expired or unused medications: Follow guidelines for proper medication disposal to prevent environmental contamination and accidental misuse.

MEANINGFUL DAY PROGRAM: Engaging daily activities designed to promote social interaction, cognitive stimulation, and physical well-being—tailored to each resident’s interests and abilities to ensure a purposeful and fulfilling day. EXERCISE PROGRAM: Structured physical activities tailored to individual abilities, focusing on strength, flexibility, balance, and overall wellness to support a healthy and active lifestyle. LAUNDRY SERVICES: Includes assistance with washing, drying, and folding clothing. Cleaning services: Regular housekeeping, including dusting, vacuuming, mopping, and sanitizing to maintain a clean and comfortable living environment.
